Welcome to the health plan finder tool. This tool will give you a general idea of which UCare plans you (or a friend or family member) may be eligible for. You must complete the form separately for each person.
Here is how it works:
- Check all of the buttons below that apply to the person for whom you are finding coverage. When you are done checking the buttons, the system will show the health care plan(s) that you may be eligible for.
- Link to each health plan listed to the right for more information about eligibility, service area, benefits, and enrollment.
Note that UCare only offers coverage programs for individuals enrolled in government-funded programs. These include Medicare and Medical Assistance. UCare does not offer commercial health care programs.
