The SEATS Program provides car seats and safety education at no charge to eligible UCare members who are pregnant (in 3rd trimester) or children up to age 8. The following documents are intended for use by contracted SEATS partners. For questions, please email SEATS@ucare.org.

Important Reminders:
Request only for distributions planned during the current month.
If a new month arrives and you did not distribute a request, then you must re-check the member’s car seat eligibility during the new month.

Important Reminders:
Report all car seat distributions online by the end of each distribution month. For example, report all January distributions by January 31st.
Distributions can only be submitted if a Car Seat Request was submitted prior to the distribution date and approved.
Distribution reporting must occur during the same month as the Request.

Review and sign the form below together with the UCare member each time you distribute a seat. This form is for your own files and does not need to be submitted to UCare.

Specific Issues with Billing UCare for Car Seat Education
If you experience problems with billing UCare for car seat education (e.g., denied claims, incorrect payments), please contact the UCare Provider Assistance Center (PAC) at 612-676-3300. To speak with a live representative about your questions/concerns:
1. Call PAC at 612-676-3300.
2. Do NOT select from the First Menu Options! Stay on the line until you get the Second Menu Options.
3. Second Menu Options: Select “2” for Claims.
NOTE: You will be prompted to enter your NPI number at this menu.
If you know your NPI, enter it.
If you do not know your NPI, press “0” or ignore this request and stay on the line until a representative picks up.
If you need to talk directly with a representative, ignore the hold time messages about “self-service” and “interactive voice response system” options.
