Medicaid plans for people
ages 0 – 64
UCare offers two health plans for adults, children and families who qualify for Medicaid (Medical Assistance) or MinnesotaCare.
Prepaid Medical Assistance Program (PMAP)
UCare PMAP is for people who qualify for Medicaid.
Eligibility
Enroll with UCare if you:
- Have Medicaid through MNsure or your county agency
- Are age 0 – 64
- Live in the UCare PMAP service area*
*Eligibility varies by county. Contact your county worker to learn more.
MinnesotaCare
UCare MinnesotaCare is for people who have lower incomes but do not qualify for Medicaid. The income limits for MinnesotaCare are higher than those for PMAP.
Eligibility
Enroll with UCare if you:
- Are eligible for MinnesotaCare through MNsure
- Are age 0 – 64
- Live in the UCare MinnesotaCare service area
