Medical Assistance (Medicaid) plans for people ages 0 – 64
UCare offers two health plans for adults, children and families who qualify for Medical Assistance (Medicaid) or MinnesotaCare.
Prepaid Medical Assistance Program (PMAP)
UCare PMAP is for people who qualify for Medical Assistance.
Eligibility
Enroll with UCare if you:
- Have Medical Assistance through MNsure or your county agency
- Are age 0 – 64
- Live in the UCare PMAP service area*

MinnesotaCare
UCare MinnesotaCare is for people who have lower incomes but do not qualify for Medical Assistance. The income limits for MinnesotaCare are higher than those for PMAP.
Eligibility
Enroll with UCare if you:
- Are eligible for MinnesotaCare through MNsure
- Are age 0 – 64
- Live in the UCare MinnesotaCare service area
