We have teamed up with Everlywell, a leading digital health company, to offer eligible members at-home lab testing kits at no additional cost. With convenient at-home screenings for certain health conditions, including diabetes, you can easily monitor and take charge of your health and wellness.

If you have questions about Everlywell or how to use your kit, call 1-855-923-2678 from 8 am – 8 pm ET, Monday – Friday. You can also visit Everlywell for more information. As always, we encourage you to talk with your provider about results and appropriate screenings.

Eligibility

UCare and EssentiaCare members are identified through claims, pharmacy or other records and sent communications on how they may benefit from completing an Everlywell kit. If you are a UCare or EssentiaCare member with Medicare or Medicaid coverage, you may be eligible to request a kit.

This program is voluntary and available at no extra cost to eligible members.
To opt out of this program and any Everlywell communications, please call the customer service number on the back of your member ID card. You can opt out of this program at any time.
