Living with heart failure can be a challenge. At‑home health monitoring makes managing your condition easier. UCare offers home heart monitoring in partnership with 100Plus, a Connect America company that provides remote patient monitoring for adults with heart failure. Best of all, it's tailored to your unique needs and offers the support of a registered nurse.

How it works

When you take a reading with any of the devices shipped to you, the data is sent directly to your 100Plus care team for review. If your measurements fall outside the normal range, the team is alerted, and they will follow up with you and notify your primary care doctor. You do not need internet access to use the equipment. The devices are ready to use right out of the box — no smartphone, app, Bluetooth or Wi-Fi is needed. Device shipping costs, batteries and test strip refills are also provided at no additional cost.

• Blood pressure cuff
• Blood glucose monitor
• Pulse oximeter
• Body weight scale

Who is eligible

If you are a UCare member 18 years old or older with a heart failure diagnosis, you are eligible for the 100Plus remote patient monitoring program. This program is available at no cost to you. Your participation is voluntary and won't impact your current plan coverage. You can opt out of the program at any time.
