Annual Health Plan Selection
Each year, between mid-October and early December, you have the opportunity to change your health plan if you want. This is called the Annual Health Plan Selection (AHPS). You may receive an AHPS letter in the mail from the Minnesota Department of Human Services (DHS). To keep UCare as your health plan, you do not have to do anything. You can disregard the letter.

UCare Health Ride
Transportation shouldn't be a barrier to good health. UCare Health Ride provides hassle-free, no-cost transportation to and from covered medical, dental and pharmacy visits.
Dental benefits
Medicaid covers medically necessary dental care, and UCare covers additional dental benefits and services. We offer support to help you understand your benefits, find rides to the dentist, schedule appointments and more.
Classes and education
Keep your mind and body sharp. UCare offers a variety of opportunities to continue learning through community education class allowances, GED education benefits and Juniper health and wellness classes.
Mental health and substance use help
Support, appointment needs and scheduling are available through the UCare Triage Line at no additional cost.
24/7 access to a nurse
Get guidance from experienced nurses on what to do and where to go when you have a health concern. The Nurse Line is open 24 hours a day, seven days a week at no extra cost.

Prescription drug coverage
Preventive medication is fully covered, and free home delivery is available through Costco Mail Order Pharmacy. Find out if your prescription drugs are covered in your plan by searching our formulary (list of covered drugs). To check if your pharmacy is in the UCare network, use the pharmacy search.
Member documents
Review PDF versions of important member documents such as your member handbook, member guide, Summary of Benefits and more.
In-network providers and clinics
See if your doctor or clinic is in your plan's network by using our online search tool. Choose your specific plan from the "Pick your plan" menu and search for doctors, hospitals and more. Narrow your search by choosing a specialty, location or other options.
Rewards and incentives
As a UCare member, you may be able to earn rewards when you take care of your health. Find out if you are eligible for a reward when you complete certain screenings, tests or exams.
Care coordination and management
If you are a member of UCare's Minnesota Senior health Options (MSHO) (HMO D-SNP), UCare Connect + Medicare (Special Needs BasicCare) (HMO D-SNP), UCare Connect (Special Needs BasicCare) or Minnesota Senior Care Plus (MSC+) your plan includes the services of a care coordinator who can help you get the health care you need.

If you are a MinnesotaCare or Prepaid Medical Assistance Program (PMAP) member, you are eligible for care management services. Our registered nurses and community health workers offer short-term, one-on-one phone support for many health concerns.
