About UCare Connect + Medicare
UCare Connect + Medicare (Special Needs BasicCare) (HMO D-SNP) is a health plan for people with Medical Assistance (Medicaid), Medicare and a certified disability.
Eligibility
Enroll in UCare Connect + Medicare if you:
- Have a certified disability
- Are at least age 18 and under age 65
- Have Medical Assistance and Medicare Part A and Part B
- Live in the UCare Connect + Medicare service area

2025 Benefits
When you join UCare Connect + Medicare, a team of health care professionals will be dedicated to helping you meet your health and wellness goals. You will be assigned a care coordinator who can:
- Identify or change your primary care clinic or primary care provider
- Coordinate transportation, dental, mental health or other benefits and services
- Provide you information about preventive care, such as physical exams, vaccinations and screenings
- Assist during transitions between care settings (such as your home, hospital or nursing home)
- Help you access programs to start exercising, quit smoking, find food or housing support, or take other steps to improve your health
Care coordination helps make sure your needs and preferences for health services are met over time.
- No premiums or deductibles*
- One member identification (ID) card for Medical Assistance, Medicare and prescription drugs
- Part D prescription drug coverage with no copays
- $60 over-the-counter (OTC) allowance loaded onto your UCare Healthy Benefits+ Visa® card each quarter to buy eligible health items including cough drops, first aid supplies, pain relief, sinus medications, toothpastes and more at participating retailers
- Earn rewards for completing certain preventive screenings, tests or exams
- Transportation at no added cost to medical, dental and pharmacy appointments
- Up to $400 per year for community education classes added to your Healthy Benefits+ card ($100 a quarter)
- Eyewear upgrades — non-glare coating, photochromic tinting and progressive lenses
- Routine foot care at no added cost once monthly
- Post-discharge medication reconciliation with a pharmacist
- Post-discharge Healthy Transitions — four re-admission prevention sessions with a community health worker
- 12 $0 visits with a caregiver advisor per year through Caregiver Assurance. Get help with care coordination, service referrals, stress reduction tips and more.
*You must continue to pay your Medicare Part B premium unless your Part B premium is paid for you by Medical Assistance of another third party.
- Access to UCare Dental Connection to help you find and schedule appointments with dental clinics that can meet your needs
- Dental care aboard the UCare Mobile Dental Clinic when it's in your area
- Additional dental coverage: two crowns per year, one crown repair per year
- One Pass fitness program with access to participating fitness locations nationwide, online classes, at-home fitness kits, brain training, no-cost social events, activities and more
- Flexible transportation — three round-trip rides per week to covered health and wellness locations and one round-trip ride per week to nutritional food allowance sites for qualifying members
- Healthy food allowance of $75 per month loaded to your Healthy Benefits+ card for members with diabetes, hypertension and lipid disorders
- Up to seven round-trip rides per week to Alcoholics Anonymous or Narcotics Anonymous
- Activity tracker plus Personal Emergency Response System (PERS) device with 24/7 call-for-help, step and heart rate tracking and built-in GPS
- Blood pressure monitor for members with a hypertension diagnosis who use the activity tracker plus PERS device
- Save with grocery discounts on pre-qualified healthy foods, with weekly discounts that are loaded to your Healthy Benefits+ card
- Medication Toolkit to make managing your medications easier
- 12 routine chiropractic visits per year, including additional exams and extremity adjustments for members with musculoskeletal disorders
- Acupuncture for acute low back pain — up to 12 additional visits per year
- Adult Dental Kit — electric toothbrush and charger, replacement brush heads, toothpaste and dental floss. One kit every three years.
- Adult Dental Refill Kit — replacement brush heads, toothpaste and dental floss. One kit per year (when you did not get the Adult Dental Kit).
- Connect to Wellness Kit — options include Fitness, Sleep Aid, Stress Relief, Smart Home Device, ADHD and Autism Support, Dental or weighted blanket
2026 Benefits
When you enroll in a UCare Connect + Medicare plan, you also get personalized care coordination and increased benefits. Your care coordinator helps to:
- Find or change your primary care clinic or doctor
- Arrange transportation, dental, mental health or other benefits and services
- Share information about preventive care, like annual checkups, vaccinations and screenings
- Ease the transition from hospital to home or nursing home, or other moves between care settings
- Support you to start exercising, quit smoking, find food or housing support, or take other steps to improve your health
- Talk to your providers to improve your care with your plan's programs
- $0 premiums, deductibles and Medical Assistance cost‑sharing*
- One member identification (ID) card for Medical Assistance, Medicare and prescription drugs
*You must continue to pay your Medicare Part B premium unless it is paid for you by Medical Assistance or another third party.
- Up to $220 per year for over-the-counter (OTC) items ($55 every three months) loaded to your Healthy Benefits+ Visa® card
- $0 rides for eligible members to covered medical, dental and pharmacy visits. Limitations apply.
- $0 glasses upgrades like non-glare coating, photochromic tinting and progressive lenses
- Rewards for taking care of your health loaded to your Healthy Benefits+ Visa card
- Post-discharge medication reconciliation with a pharmacist
