Receive support for your mental health or substance use needs

Collaborate with your case manager to stay healthy

The UCare Case Management program provides comprehensive support with clinically-trained case managers for UCare members with mental health or substance use concerns. Our case managers play an active role in your health journey by working with you, your caregivers and providers to identify quality mental health or substance use services that you need. They can assist with transitions of care, connect you to specialty services and provide general support when you need it.

Get help through the UCare Triage Line

Our mental health and substance use services team will help connect you with referrals to mental health or substance use case management and other services through our triage line. You can reach out in two ways:

How it works

Case managers build strong relationships with health plan members to understand their individual needs and health goals.

They will work with you to:

  • Complete a health assessment over the phone
  • Create a personalized plan
  • Connect you to available resources and support to achieve your desired health outcomes
  • Assist with finding resources for basic needs such as food, housing and transportation

They can also help you if your situation changes like a hospitalization. Case managers can become resourceful advocates, monitoring your progress, adjusting your plan as needed and making referrals to the support services. All this to empower you along your journey to health and wellness.

You may be eligible

UCare members who need assistance with mental health or substance use are eligible for case management services. You can choose to opt out at any time during the program.

The release of information form allows UCare to share your medical information and help our case managers coordinate with other providers to give you the best care available. Select your plan below and complete the form.
