Advocating for your child’s health and wellness
The UCare Child and Adolescent Case Management program provides free support from clinically-trained case managers. Our case managers will work with you and your child’s care team to support you as you navigate their health and wellness goals.
Get help through the UCare Triage Line
Our mental health and/or substance use services team will help connect you with referrals to mental health and substance use case management and other services through our triage line. You can reach out in two ways:

How it works
Our clinically-trained case managers are dedicated to offering a range of support, including referrals to specialty services for kids diagnosed with ADD or ADHD, autism, mental health or substance use concerns. They work with your child and the family to complete an assessment and create a care plan tailored to your child’s needs and health goals. They ensure all the pieces of your child's care plan work together by:
- Acting as a central point of contact
- Advocating for your child's health
- Locating community support and resources for basic needs such as food, housing and transportation
- Coordinating appointments between health providers and more
Who is eligible
Child and adolescent case management services are available to children with a UCare health plan who need additional support with the following:
- ADD or ADHD
- Autism
- Mental health or substance use
We work with family members who may not be UCare members to help coordinate their child's care. You can choose to opt out of this program at any time.
