About UCare Connect
UCare Connect (Special Needs BasicCare) is a plan for people with a certified disability who qualify for Medicaid (Medical Assistance). It has additional benefits that are not available on regular Medicaid.
Eligibility
Enroll in UCare Connect if you:
- Have a certified disability
- Are between the ages of 18 and 65
- Have Medicaid with or without Medicare Parts A and B*
- Live in the UCare Connect service area
*If you have Medicare Parts A and B, you may be eligible for UCare Connect + Medicare (HMO D-SNP).

When you join UCare Connect, a team of health care professionals will be dedicated to helping you meet your health and wellness goals. You will be assigned a care coordinator who can:
- Identify or change your primary care clinic or primary care provider
- Coordinate transportation, dental, mental health or other benefits and services
- Teach you about preventive care, such as physical exams, vaccinations and screenings
- Assist during transitions between care settings (such as your home, hospital or nursing home)
- Help you access programs to start exercising, quit smoking, find food or housing support, or take other steps to improve your health
Care coordination helps make sure your needs and preferences for health services are met over time.
- Keep Your Coverage team of specialists that guide you through the Medicaid renewal process so you can avoid breaks in care and services
- Access to UCare Dental Connection to help you find and schedule appointments with dental clinics that can accommodate your needs
- Ability to join or leave the health plan whenever you like (if you choose to leave, you will be enrolled through the last day of the month)
- No-cost transportation to and from covered medical, dental and pharmacy appointments through UCare Health Ride
- Nurse Line — members can call 24/7 for reliable health information from nurses who offer health advice and provide answers to health questions
- Food access referrals to connect with local food resources through Second Harvest Heartland
- Personal Care Assistance, Home Care Nursing Services and disability waiver benefits for those who qualify
- Dental care aboard the UCare Mobile Dental Clinic when it's in your area
- Dedicated providers in the UCare Connect network who understand and know how to treat your unique needs
- One Pass® fitness program with gym membership, online classes and more
- Connect to Wellness Kits — options include Fitness, Sleep Aid, Stress Relief, ADHD and Autism Support, Dental or Smart Home Device. One kit per year.
- Up to $400 per year for community education classes ($100 every quarter) loaded to your UCare Healthy Benefits+ Visa® card
- Save with grocery discounts on healthy foods at participating grocery stores. Weekly discounts are pre-loaded onto your Healthy Benefits+ card.
- Earn rewards loaded to your Healthy Benefits+ card for completing certain preventive screenings, tests or exams
